Police Checks

What these are for:

LifeSwitch carries out Police Checks for those who work directly with children and youth. We require a Police Check for anyone aged 17+ who is dealing directly with children in any of our kids or youth programmes and during Camp Survivor. A check includes sighting approved forms of ID (these are listed below).

A Police Check expires after three years, if yours has expired we will let you know and ask you to complete and submit another form.

Options for completing:

- Complete it digitally and email it back to laurel@lifeswitch.org.nz with a photocopy/photo of two forms of ID (see list of below).
- Print it, complete it and drop it off to The Hub desk in the foyer on Sunday and bring your ID with you for a LifeSwitch staff member to sight.
- Visit The Hub desk in the foyer on Sunday to collect a printed form, complete it there, and show your ID to a LifeSwitch staff member.

Please note that you just need to complete SECTION 2 (pages 3 & 4) - a LifeSwitch staff member will fill in pages 1 & 2.

Forms of ID accepted:

We are required to see one Primary Form of ID and one Secondary form. Below is a list of acceptable forms of ID:

Primary IDs include:
• Passport (NZ or Overseas)
• NZ Firearms Licence
• NZ Full Birth Certificate (issued on/after 1998*)
• NZ Citizen Certificate
• NZ Refugee Travel Document
• NZ Emergency Travel Document
• NZ Certificate of Identity

Secondary IDs include:
• NZ Drivers Licence
• 18+ Card
• NZ Full Birth Certificate (issued before 1998)
• Community Services Card
• SuperGold Card
• NZ Employee Photo Identification Card
• NZ Student Photo Identification Card
• Inland Revenue number
• NZ issued utility bill (issued within 6 months)
• NZ Teachers Registration Certificate
• NZ Electoral Roll Record
• International Driving Permit
• Steps to Freedom Form
